1.  An Important Note About Children.  Children (persons 13 years old or younger) are not eligible to use this web site unsupervised, and we ask that children do not submit any personal information to us.

2.  Information We Collect.  In general, you can browse the web site without telling us who you are or revealing any personal information about yourself. Once you give us your personal information, you are not anonymous to us. If you choose to register for an account or to log into an account, we will collect personal information about you. In order to establish your account, you must provide us with your email address.

Under some circumstances, we may require some additional information from you, such as, but not limited to:

if you are making a payment through the web site, we use your financial information, including credit card information, to verify the accuracy of your name, address, and other information, as well as to allow you to make the payment;

if you request an appointment or appointment change, we require a telephone number and/or email address so we may contact you;

if you use our appointment center, we will collect and display your entire appointment history including date of appointment, appointment time, type of appointment and status;

you also may be asked to provide us with your home address, home phone number, cell phone number, work phone number, email address and portions of your birth date when applicable; and

if you send us emails relating to personal matters, appointments, or communications, we may collect such information into a file specific to you.

In addition to information that you provide us, we automatically track certain information based upon your behavior on the web site. We use data collection devices, such as “cookies”, on certain pages of the web site to help analyze our web page flow, measure promotional effectiveness, and store your user preferences. By way of example (without limiting the foregoing), we may be forced to disclose personal information to the government or third parties under certain circumstances or third parties may unlawfully intercept or access transmissions or private communications. Therefore, although we use industry standard practices to protect your privacy, we do not promise, and you should not expect, that your personal information or private communications will always remain private.

5.  Control of Your Password.  You are responsible for all actions taken with your User ID and password, including fees without limitation charged to your account. Therefore we strongly recommend that you do not disclose your User ID and password to any third parties. YOU ARE RESPONSIBLE FOR ALL ACTIONS TAKEN WITH YOUR ACCOUNT, EVEN IF YOU CHOOSE TO SHARE YOUR USER ID AND PASSWORD OR YOUR PERSONAL INFORMATION WITH THIRD PARTIES. If you lose control of your password, you may lose substantial control over your personal information and may be subject to legally binding actions taken on your behalf. Therefore, if your password has been compromised for any reason, please change the password immediately by visiting the web site’s home page and requesting a new one.

6.  Accessing, Reviewing and Changing Your Personal Information.  We offer the ability to review and change the information you submit to us by logging into the web site and entering in the new information yourself. You must promptly update your personal information if it changes or is inaccurate.

Upon your request, we will deactivate your account, contact information, billing information, and financial information from the 4YOURSERVICE.COM active databases. To make this request, please visit the web site’s home page and click on the Disable Account link. Your account information will be deactivated as soon as reasonably possible based on your account activity and in accordance with our deactivation policy and applicable law. Your account will automatically deactivated within approximately five days after you have been dismissed from treatment.

Even after deactivating your account, we may retain in our files some personal information for our operations including without limitation to prevent fraud, collect any fees owed, resolve disputes, troubleshoot problems, assist with any investigations, enforce our User Agreement/Terms of Use and comply with legal requirements as is permitted by law. Therefore, you should not expect that all of your personal information will be completely removed from our databases in response to your requests.

7.  Security.  We use procedural and technical safeguards to protect your personal information against a loss or theft as well as unauthorized access and disclosure, including “firewalls” and Secure Socket Layers. ALTHOUGH WE EMPLOY MANY DIFFERENT SECURITY TECHNIQUES TO PROTECT SUCH DATA FROM UNAUTHORIZED ACCESS BY USERS, ABSOLUTE SECURITY DOES NOT EXIST ON THE INTERNET.

8.  Changes to this Privacy Statement.  We may amend this privacy statement at any time by posting the amended terms on the web site. All amended terms shall automatically be effective 10 days after they are initially posted on the web site.
